Founder/Operator

Tom holds a Masters of Public Administration in Healthcare Services Management and a Bachelors of Arts in Psychology. Tom has over 25 years experience working with geriatric and disabled populations in various care settings and in the community. Tom previously worked for 9 years as the Director of Client Services at Midwest Care Management Services in Chicago, which provides care management, guardianship and POA services to disabled adults and children. Through this experience, Tom has gained a wealth of knowledge and has developed a network of associates in the areas of probate law and estate/wealth/trust management. Tom also has developed a wide network of resources for homecare, home health, and medical related services & providers throughout Chicagoland.

Tom has 10 years direct experience working as a licensed nursing home administrator at multiple nursing homes in New Jersey and Illinois. Through this experience, Tom has gained an extensive knowledge of geriatric care needs and issues & illnesses commonly impacting seniors, as well as nursing home regulations and resident rights. Tom is well-versed in navigating the long-term care system in order to ensure clients are receiving the optimal level of care and support. This knowledge expands to other care settings, such as assisted living and memory care, as well.

Tom has a wealth of experience working with multiple populations, including individuals with mental illness, traumatic brain injury, intellectual disability, dementia & geriatric-related illness, as well as autism spectrum disorder and other developmental disabilities. Tom has extensive experience managing programs and care settings for the psychiatric population. Tom has nearly 15 years experience managing group homes, psychiatric wards, and nursing homes with large psychiatric populations. Tom also has over 5 years experience in the field of neuro-cognitive rehabilitation therapy, working with TBI patients to maximize their potential for autonomy in home and work settings.

Tom is also a veteran of the U.S. Army and he speaks Spanish.

Admin/Estate Manager

Stephanie has an Associates Degree in Business Administration. She is a dedicated Estate Manager with a passion for process efficiency, and brings a blend of expertise and a commitment to excellence to her role. With a keen eye for detail and 9 years of experience in Guardian and POA Management, Stephanie's resourcefulness helps people to navigate the intricacies of Estate Management, which can involve a plethora of tasks that range from Asset Investigation, Bill Payment, Asset Liquidation, Support with Social Security Benefits, Power of Attorney Services and many more. With a focus on client well-being, she crafts tasks thoughtfully, always considering the unique needs and perspectives of those she serves.

Prior to joining the team, Stephanie worked for 9 years with Midwest Care Management Services in Chicago. During her tenure with Midwest, she served in many roles, including Office Coordinator, Estate Manager, and most recently Operations Manager. In this operations role, she was responsible for all of the administrative aspects of the organization, including management of the office/admin team. She also continued to act as decedent administrator on behalf of the company.

Care Manager

Santyna holds a Masters of Arts in Social Service Administration and a Bachelors of Arts in Psychology. She is a Licensed Clinical Social Worker (LCSW) and has over 10 years of experience with a wide range of populations and settings. Santyna has over 5 years of experience with care management, guardianship and POA services with adults and children who are diagnosed with cognitive and physical disabilities and adults diagnosed with chronic mental health illnesses. 

Santyna currently works as a psychotherapist in a private practice in addition to working at TD Care Management. 

Santyna also speaks Spanish.

Psychosocial Care Manager

Support Staff

Griffin provides administrative and technical support with some of the office operations at TD Care Management & TD Guardianship. He is the go-to guy for setting up office tech, assembling furniture and other items for the office and for clients, and also provides support to our care managers in the field when an extra set of eyes and hands is needed. Griffin is most passionate about assisting our clients with navigating their home tech (computers, cell phones, tablets) and enjoys listening to clients sharing their stories. Griffin is currently studying video game design and graphic arts coursework online with Virginia Wesleyan University, and hopes to pursue a degree in this field.

Griffin is on the autism spectrum, so brings a unique perspective to the team. Griffin also speaks some Japanese.
