Geriatric Care Management
At TD Care Management, we believe the golden years should be the best years. We have extensive experience working with older adults who need additional support at home, or assistance with identifying an alternative care setting. We particularly work with adults with dementia and neurocognitive disorders, and are sensitive to their needs as the disease progresses over time. Due to our strong working relationship with various service providers and care facilities in the Chicagoland area, we are able to tailor a care plan to best suit the individual’s needs and adjust the plan whenever necessary.
Nursing Home Patient Advocacy
TD Care Management is highly knowledgeable in Federal nursing home regulations and nursing home resident rights. This knowledge allows us to quickly and effectively advocate on behalf of our clients who are residing in nursing homes (and other long-term care settings). We work with you, your family, your healthcare providers, and the facility staff to ensure that you receive the proper care and treatment throughout your nursing home stay, and that your autonomy and quality of life are at the forefront of your nursing home experience.
Care Management
TD Care Management is highly experienced in providing care management services to disabled individuals of all ages and functioning levels. We work with trust officers and probate attorneys to implement top quality care plans for our clients and to maximize resources available to them. We have specialized experience working with clients following a personal injury settlement and conducting a needs assessment to ensure that all the client’s needs are being met. We have specialized experience working with the mentally ill, as well as the intellectually and developmentally disabled populations (ID/DD). We serve clients of all ages and ability ranges.
Powers of Attorney
We offer power of attorney (POA) services to individuals who are trying to prepare for their golden years and recognize their need for help in managing their healthcare and finances, or who are finding themselves already in the process of an age-related or illness-related decline. We are able to offer flat monthly or annual rates for springing powers of attorney, as well as hourly rates to tailor to your specific needs.
Intensive Case Management
We have experience managing complex medical issues and developing & implementing care plans in collaboration with medical providers throughout the recovery process. If you or a loved one has experienced a sudden illness, accident, or debilitating disease, TD Care Management can assist you in navigating the continuum of care from hospital and acute care settings, to post-acute and sub-acute settings, to long-term care or home settings. We will assist you in maximizing the services available to you throughout your recovery process.
Veteran Services
We are able to assist clients who have served in the military by securing any veteran’s benefits for which they may be eligible and ensuring that they are treated with the respect they deserve as they navigate the long-term care continuum. TD Care Management is proud to provide service to our military veterans at a 25% discounted rate.
Psychosocial Care Management
We aim to help individuals with mental illnesses to connect with the appropriate inpatient or outpatient services in order to better manage their mental health. Our compassionate and professional care managers have a solid background in behavioral health services. We are able to provide some respite and support to family members who are trying to assist their struggling loved one through these processes.
Guardianship Services
Guardianship services are now being offered through our partner company, TD Guardianship NFP. Guardianship of Person and Guardianship of (small) Estate, as well as Temporary and Limited Gaurdianship services are available to disabled adults who no longer have the capacity to make their own personal healthcare and/or financial decision. We are experienced in navigating difficult family dynamics that may lead to the need for a 3rd party guardian. We are sensitive to meeting the needs of our clients while including input from family and friends.
